A model for family physicians providing longitudinal care

The Longitudinal Family Physician (LFP) Payment Model was developed by the BC Ministry of Health in consultation with Doctors of BC and BC family physicians. It was launched in 2023 in response to an urgent need to retain and attract family physicians by making longitudinal family practice sustainable and rewarding.

The payment model is a key foundational building block to help us create a high-performing primary care system and is a step toward helping to make longitudinal family practice stable, sustainable, and rewarding so that more British Columbians will be able to access care from a family physician. It is grounded in a commitment to increase patient access to community-based, longitudinal family medicine care, and expand primary care capacity across British Columbia. To reflect the comprehensive and continuous nature of family medicine, it is a blended payment model that compensates physicians for time, interactions, and their overall patient panel.

Panel payment methodology change

On March 31, 2026, it was announced that the LFP panel payment and FPSC CLFP payment (for fee-for-service and applicable AP contracts) are transitioning to a new calculation methodology expected to start with the installment payable on November 30, 2026. Read the full announcement.

Under the new methodology, panel payments will be calculated using: 

  1. Your panel registry in the Provincial Attachment System (PAS) to measure panel size.
    • Patients on your PAS panel with a PAS MRP status of Confirmed will be included when calculating your panel payment under the new methodology. A snapshot of your PAS panel is taken at the end of each payment period—see table below for cutoff dates to update your PAS panel. 
    • Patients with the PAS MRP statuses of Pending, Not the MRP, or Removed will be excluded.
    • Patients with a PAS MRP status changing from Confirmed to Deceased will be included for the payment period in which they pass away. Deceased patients will be excluded in all subsequent payment periods.
    • Only clinics where you practice under a payment model that is eligible for the LFP panel payment or CLFP Payment are included.

Payment and calculation details

The expected first payment calculated under this new methodology will be paid to you on November 30, 2026, for the payment period of July 1 to September 30, 2026.

Next steps before October 1, 2026

  1. To ensure your panel payment is calculated accurately, review your PAS panel to ensure it aligns with your list of empanelled patients. We advise that you log into PAS with your OneHealthID at your earliest convenience. If you encounter issues, we want to support you early in the process. Learn more about PAS.
  2. To ensure your patient complexity is reflected accurately, continue to follow best practices when submitting ICD-9 codes in your MSP billings. Learn more about ICD-9 codes (login required).
