- Any individual can request a claim form. To ensure privacy, we provide information about the insured and their coverage only to authorized individuals.
- A claimant may designate an individual to assist with the claims process and can authorize Doctors of BC to communicate directly with their representative by completing the consent-to-share form.
- Doctors of BC staff are pleased to assist your claim submission by confirming your active coverage, clarifying the claims process, and providing guidance on required documentation and form completion.
Accidental Death and Dismemberment
Start your claim
Eligibility: An insured physician, spouse, or dependents with active coverage who have died directly due to an accident or have become injured in a manner covered by the policy may be eligible for a benefit.
- To request an Accidental Death and Dismemberment claims package, please email insurance@doctorsofbc.ca.
The claims package will typically include the following documents:
- Claimant statement
- Proof of death documentation
- Attending physician’s statement
Submit your claim
- Please email your completed claims package to claims@suttonspecialrisk.com. A Sutton claims adjudicator will contact you directly about your claim.
Critical Illness
Start your claim
Eligibility: An insured physician, spouse, or child with active coverage who has been diagnosed with a covered illness and has met a specified survival period may be eligible for a benefit.
- To request a Critical Illness claims package, please email insurance@doctorsofbc.ca.
- Manulife will contact you about your diagnosis and send you a claims package. If you are also making a disability claim, we will coordinate your claims packages.
Submit your claim
- Follow the instructions in your claims package to submit your claim to Manulife directly. A Manulife claims adjudicator will contact you directly about your claim.
Disability Income Protection and Professional Expense
Start your claim
Eligibility: An insured physician with active coverage who cannot work due to an accident or illness may be eligible for a benefit.
- To request a claims package for Physicians’ Disability Insurance (PDI), INCOMEprotect™ Disability Insurance, and/or Professional Expense Insurance, please email insurance@doctorsofbc.ca. If you are eligible to file claims for more than one plan, your claims will be coordinated. Please ensure that your claim is submitted within 90 days of satisfying the elimination period (refer to your certificate for details).
The claims package will typically include the following documents:
- Claimant statement
- Attending physician’s statement
- Employer’s statement (you complete this form if you’re self-employed)
- Direct deposit or void cheque form
- Email consent form
- Communications form

Health and Dental (HBTF)

Eligibility: Physicians, staff, or their dependents can make claims under the plan if they are actively enrolled in the Health Benefits Trust Fund (HBTF).

Life
Start your claim
Eligibility: If the insured physician or spouse dies, their beneficiary(ies) may be eligible for a benefit.
The claim form can be completed by the beneficiary, executor, or signing officer if the beneficiary is a corporation.
- To request a Life Insurance claim package, email insurance@doctorsofbc.ca. Please include your relationship to the deceased member and provide a copy of the will if you are the executor of the deceased member's estate.
The claims package will typically include the following documents:
- Claimant statement
- Proof of death documentation
- Attending physician’s statement
