Eligibility: An insured physician, spouse, or dependents with active coverage who have died directly due to an accident or have become injured in a manner covered by the policy may be eligible for a benefit.

• To request an Accidental Death and Dismemberment claims package, please email insurance@doctorsofbc.ca.

The claims package will typically include the following documents:

• Claimant statement
• Proof of death documentation
• Attending physician’s statement

Submit your claim

• Please submit your forms directly to Sutton Risk as instructed in your claims package. A Sutton Risk claims adjudicator will contact you upon receiving your documents.

Eligibility: An insured physician, spouse, or child with active coverage who has been diagnosed with a covered illness and has met a specified survival period may be eligible for a benefit.

• To request a Critical Illness claims package, please email insurance@doctorsofbc.ca. 
• Manulife will contact you about your diagnosis and send you a claims package. If you are also making a disability claim, we will coordinate your claims packages.

• Please submit your forms directly to Manulife as instructed in your claims package. A Manulife claims adjudicator will contact you within 10 business days of receiving your documents.

Eligibility: An insured physician with active coverage who cannot work due to an accident or illness may be eligible for a benefit.

• To request a claims package for Physicians’ Disability Insurance (PDI), INCOMEprotect Disability Insurance, and/or Professional Expense Insurance, please email insurance@doctorsofbc.ca. If you are eligible to file claims for more than one plan, your claims will be coordinated. Please ensure that your claim is submitted within 90 days of satisfying the elimination period (refer to your certificate for details).

The claims package will typically include the following documents:

• Claimant statement
• Attending physician’s statement
• Employer’s statement (you complete this form if you’re self-employed)
• Direct deposit or void cheque form
• Email consent form
• Communications form

• Please submit your documents directly to Manulife as instructed in your claims package. A Manulife claims ajudicator will contact you within 10 business days of receiving your documents.

For claims incurred on July 1, 2026 and later with Manulife

Submit online

Register for the Manulife Benefits site:

1. Download the Manulife app or set up your Manulife ID and sign in through the Manulife Group Benefits site. Watch this 2-minute video on how to register for the Manulife site. 
   • Download from the Apple App Store

2. After setting up your Manulife ID, you’ll need to activate your group benefits plan by registering your personal information and new plan numbers:
   • Plan contract number: 155849
   • Certificate number: XXXXXX
3. At the end of the setup process, click "Go" to access the site, then navigate to the "My Account" tab to review and/or update personal details and opt in to direct deposit for faster reimbursements. 
   • For technical support, call Manulife at +1 800-268-6195 (Monday–Friday, 5:00 a.m.–5:00 p.m. PST).

For claims incurred before July 1, 2026 with Canada Life

Canada Life claim guides and forms:

As the HBTF plan has transferred to the new Health and Dental plan, you have until October 28 to submit claims incurred prior to July 1 to Canada Life by using the health claim form or dental PDF claim form. The Canada Life benefits site is now closed for online claims submissions.

Cost-Plus claim guides and forms

How do Cost-Plus claims work? Example: Canada Life covers prescription drugs at 80% once the annual deductible has been satisfied. The 20% that Canada Life does not cover, plus the annual deductible, can be claimed through your Cost-Plus plan.

You would submit the portion of the claims not reimbursed by Canada Life on the Doctors of BC Cost-Plus claims website for processing. Doctors of BC will debit the submitted claims, plus the 7% administration fee, from your corporate bank account. The following week, a deposit in the amount you claimed—less the administration fee—will be deposited into your personal bank account.

Eligibility: If the insured physician or spouse dies, their beneficiary(ies) may be eligible for a benefit.

The claim form can be completed by the beneficiary, executor, or signing officer if the beneficiary is a corporation.

• To request a Life Insurance claim package, email insurance@doctorsofbc.ca. Please include your relationship to the deceased member and provide a copy of the will if you are the executor of the deceased member's estate.

The claims package will typically include the following documents: 

• Claimant statement
• Proof of death documentation
• Attending physician’s statement
