'Train the Mentor' program engages the medical community

Early last month, 35 physicians knowledgeable and experienced with polypharmacy risk reduction came together for an interactive one-day ‘Train the Mentor’ session. The ‘Train the Mentor’ program is engaging the medical community through a variety of approaches to tackle the risks of taking multiple medications for elderly and frail in residential and acute care. The program is part of the Shared Care Polypharmacy Risk Reduction initiative.

Much like a relay race, the session encourages these physicians to pass the baton and mentor colleagues in their communities to promote strategies for effective medication reviews for their patients.

Eight dynamic speakers, including physicians, a pharmacist and nurse specialist, provided their unique perspectives and approaches to medication reviews, as well as clinical tools and resources. Role-playing, and interactive group discussions using case studies helped to demonstrate effective communication strategies with family members and other care providers around the following topics:  

  • Ensuring early involvement of families in discussions around end of life care
  • Utilizing clinical tools and resources for effective polypharmacy risk reduction
  • Strategies for effective interdisciplinary medication reviews

Despite taking place on a sunny Saturday, attendees were happy to be there and found the session worthwhile - feedback stated that “the program exceeded expectations”.  Physicians also shared how the session impacted their long-term thinking and changes they would be making to their practice as a result:

  • “try and involve care aide/RPN’s more in care conferences”
  • “I have more confidence in setting up mentoring of interested physicians and plan to start this activity”
  • “Investigate some of the tools for polypharmacy management”
  • “Improve connection with family regarding treatment, medications and long term goals”
  • “Be more sympathetic and more realistic”
  • “I will increase my overall engagement in residential care as a result of renewed interest and confidence”

The Polypharmacy Risk Reduction Initiative will continue to engage physicians and care teams. With 35 new physician mentors on board, the extended team will be in a favorable position to spread awareness and knowledge for further polypharmacy risk reduction across the province.
