2024 Shared Care Partners in Care Event

This invite-only event includes two and a half days of sessions, activities, and social events.
Please note that day 1 is for project leads/project managers only. Physicians and health care partners are invited to attend from Day 2 onwards. Read through the agenda to learn more about what to expect during this three-day event.
