These resources are intended to support family physicians and nurse practitioners providing longitudinal care to develop and submit their initial list of empanelled patients to the Patient Attachment System (PAS), as well as keeping their PAS panel up to date. There are also resources to support physicians on the LFP Payment Model use their EMR to enter LFP payment codes.

The resources below provide instructions on how to create your initial list of empanelled patients and submit using bulk claiming functionality in your EMR, or third-party billing system.

Ongoing panel updates
After submitting your list, it is important to keep your PAS data up to date. Please refer to this guide for EMR workflow considerations to keep your PAS panel list accurate.
Routine panel management in your EMR and updates to your PAS data should be integrated to ensure that both accurately represent your patient panel.
